No Guardian against massive fraud

Yesterday (Jan. 14), Minnesota Attorney General Keith Ellison announced a new fraud prosecution. His press release,

So, add another $3 million (with an “m”) onto the multi-billion-dollar fraud pile. Details,

Attorney General Keith Ellison announced that his Medicaid Fraud Control Unit (MFCU) has charged Mohamed Abdirashid Omarxeyd with eight counts of felony theft by false representation for using his Minneapolis-based agency, Guardian Home Health Services, to bilk the Minnesota Medical Assistance (Medicaid) program out of over $3 million.

Ellison alleges that the fraud occurred over a four-year period and involves the personal care assistant (PCA) program, among other programs operated by the state Dept. of Human Services (DHS). According to DHS records, Guardian was located in south Minneapolis, was closed at the end of July 2025, and had a long history of violations dating back to 2019.

The case filed against Omarxeyd, age 57 of Minneapolis, appears to be the first fraud case initiated by Ellison in over a year.

Ellison’s most recent effort in the courtroom failed in spectacular fashion when a jury’s unanimous guilty verdict was tossed by a Hennepin county judge, because reasons. By coincidence, this latest case was also filed in Hennepin County. So far, the newest case is being handled by a different judge.

As they say, try try again.

State court records indicate that the case against Omarxyed is related to another fraud case filed the same day, against one Maryan Haji Garad, age 41. Garad was a manager at Guardian. Between the two they are together charged with fourteen (14) counts, all felonies. The dollar amount allegedly stolen by Omarxyed stands above $3.2 million and at $540,000 for Garad. Let’s call it an even $4 million.

Omarxyed and Garad are both scheduled to make their first court appearances in the case on February 3.
